Once you have reviewed the instructions
to proceed to online forms
Please have your student’s ID number available as it is required on each form!!
DO NOT USE ATHLETE NICKNAMES ON FORMS
1) Please make sure all information is accurate, as this will be used in case of emergency.
2) Parent/Guardian AND Student signatures are required in order for forms to be accepted electronically.
3) To access the required athletic participation forms, place your cursor over the “Electronic Participation Forms” tab to view the drop down menu.
4) Click on the form name and fill out the information requested. If you are unsure about the answer to a question, please answer not applicable, unknown or none.
5) To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box.
6) Once you have completed each form, you will have the opportunity to print the document.
7) You will receive a confirmation email once the documents have been reviewed by the Aledo ISD Athletic Trainers.
8) Write N/A in any required fields that do not apply to you. All fields must have something in the box.
9) Once completed, click on the “Medical History/Physical” tab to fill out and print the physical page to take with you to the doctor.
* All athletes in 7th through 12th grade must have an annual physical examination by a medical doctor to participate in practices or games.
* Note for physicals: Students are required to use the Pre-participation Physical Examination Form provided by the UIL. NO OTHER Physical Examination Form will be accepted. A new physical exam must be given prior to each school calendar year. Physicals are good for 1 year, and will expire 1 year from the date it was completed. A new physical must be completed every year in order to maintain eligibility. Students will not be allowed to participate if their physical has expired. The physical form can be accessed from the Medical History/Physical tab. You are encouraged to fill out the medical history portion of the form prior to printing.
Parents are NOT able to upload Physicals and Medical History forms to Rank One, but may be emailed as a .pdf to the Athletic Training Staff at your child’s school. The physical must be completed by a physician and must be returned to the Athletic Trainers or the junior high head coach.
